Petition or Notification Regarding Change of Advisor

  • Students: Use this form to request or provide notification of a change in your faculty advisor. If you have already constituted your dissertation committee, you must use this form to petition M&C for approval of the change; at any time before that, use this form to provide notification of the change. If you are using this form as a petition, you will be informed by e-mail and/or mail of the M&C faculty's decision.

    IMPORTANT NOTE: IN ALL CASES make sure you have discussed the change with all of the people involved - a form notification is not the appropriate way to communicate with faculty!

    DISTRIBUTION: The information in this form will be automatically relayed to your current and new advisors and the M&C program director (who must subsequently approve it if you have constituted your dissertation committee). You will also receive a copy of the information that is sent for your records.
